Services are available under the Brain Injury (BI), Community Mental Health Supports (CMHS), Children’s Extensive Support (CES), Developmental Disabilities (DD), Elderly, Blind, and Disabled (EBD), Money Follows the Person (MFP), and Supported Living Services (SLS) waivers, based on eligibility.

How to request NMT:
1. Contact your case manager (or health plan/transportation broker, if applicable)
2. Ask about Non-Medical Transportation (NMT)with A+ Angels LLC
3. Once approved/scheduled, we coordinate the ride details

A+ Angels LLC provides Individual Residential Services & Supports (IRSS) for eligible adults with disabilities who live in their own home or apartment. IRSS is Medicaid-funded when approved and focuses on helping members

IRSS supports may include:
• Daily living skills (routines, housekeeping support, meal planning)
• Safety planning and wellness checks (as authorized)
• Support with community integration and independence goals
• Coordination with the team (case manager and supports plan)
How to start IRSS services:
1. Reach out to your case manager and ask about IRSS eligibility
2. If authorized, we complete intake and coordinate supports
3. Services begin based on the approved plan
